The Practice Prescriptions
For your convenience please complete the prescription order form and hand or post into reception 48 hours in advance. This is important for safety and accuracy of prescriptions. When requesting prescription please check that you have requested all the medications needed for the duration of your prescription.
If you have excess or there is an item which is not needed please advise. Meticulous care is needed in ensuring quality control of the prescribing system and we ask for your support in ensuring your safety.
