Letters & Forms
Certificates:
Requests for repeat Social Welfare or Work Certificates can be made to reception. Requests may be made by:
- Phone: 029 60636
- Fax: 029 61170
- Post: Enclosing a stamped addressed envelope.
Certificates will be prepared each evening and will be ready for collection on the next working day. We regret that we are forbidden in legislation to issue certificates where the patient has not been seen.
Forms / Letters:
Patients will often require forms to be filled out by the doctor. Whilst such needs are important to us as well as yourselves, we do have to prioritise these requests with the medical needs of our patients. Please give us a minimum of 5 working days notice for the completion of these forms. There may be an administrative charge.
Medical Card Support Letters:
Your medical card denotes the expiry date. Should you need the doctor to supply a letter of support, please give a minimum of 14 days notice in advance of this date.
